A change order signed after the work is done isn't an agreement. It's an invoice the client can argue with, because "while you're in there, could you also" needs a price and a signature before anyone picks up a tool. A change order adjusts two things: the contract sum and the contract time. Teams settle the sum and leave the days blank, so the crew still owes the client the original completion date, even after the client added a week of work. Typeform's change order form records the scope, the cost, and the schedule impact together, before the work starts.

With a sample change order form, you describe the change and what it does to the contract: added scope, revised cost, revised dates. Conditional logic reads whether the change adds work, substitutes something, or removes it. Added work opens the labor, the materials, and the days it pushes the schedule. A substitution or a deletion opens the credit and asks whether the original completion date still holds. The client approves before the crew starts.

Each change order sits with the original contract as a dated amendment carrying the revised total and the revised completion date. When the job runs long, you can show which approved changes added the days. The final invoice matches a run of signed approvals instead of a conversation two people remember differently.

Sample Change Order Form FAQs:

A change order form is an online form for documenting a change to an agreed scope of work, along with what it does to the price and the schedule. It collects a description of the change, the revised cost, the days it adds or removes, the reason for it, and the client's approval. Contractors and project managers use change order forms to agree a change in writing before the work starts rather than after it.

Without a change order form, you agree to changes on site and invoice them at the end. The client remembers approving the extra outlet but not the fifteen hundred dollars it added, nobody wrote down why the schedule slipped, and now you're negotiating over a job that's already finished. A change order form puts the price and the new date in front of the client before the work starts, while they can still say no.

Capture what the change affects:

  • Description of the change and why
  • Revised cost, itemized
  • Days added or removed
  • Revised completion date
  • Client approval and date

Yes. Typeform variables can add the change amount to the original contract sum and show the revised total on the approval screen, so the client sees the figure they're agreeing to instead of a line item they have to add up themselves. Pass the original sum in with a hidden field from your project software, so nobody retypes it and nobody transposes a digit. Do the same with the days: total the extension against the days already approved, and show that number beside the new completion date the client is signing off.

Yes. You can upload photos of the condition that prompted the change with File Upload, available on paid Typeform plans. You can add images or PDFs up to 10MB each, so the rot behind the wall is on record next to the cost of dealing with it. Every photo arrives attached to the change order.
