Student Attendance Form
Move past a form that logs a check mark and calls it done. Confirm what attendance data can actually be shared without consent, and document absences the way your state's truancy law actually measures them.
Roll call, paper sign-in sheets, and manual tallies drain valuable class time—and still leave gaps. Absences are missed, tardies are recorded inconsistently, and when a pattern finally needs attention, the details are scattered across notebooks and spreadsheets.
Typeform’s student attendance form keeps everything in one place. Capture each student’s name, class, date, and attendance status—present, absent, late, or excused—in seconds. Conditional logic streamlines check-in: present students move straight ahead, while an absence or late arrival triggers a quick follow-up for the reason.
Customize the form for any classroom or campus by adding fields for minutes late, remote attendance, or absence notes. Share it via a QR code at the door, embed it in your LMS, or send the link to a substitute. Responses sync with Google Sheets in real time, keeping attendance records current without exports or end-of-day counting.
A student attendance form records whether a student was present, absent, or tardy on a given day, along with whether an absence was excused, and any documentation supporting that. It's the underlying record that daily attendance tracking, truancy determinations, and reporting all depend on.
It depends on what's being shared. FERPA treats "dates of attendance," the general period someone attended, like a school year, as directory information a school can disclose without consent. But that term specifically excludes daily attendance records, whether a student was present or absent on a particular day, which remain protected and generally require consent to disclose beyond school officials with a legitimate educational interest.
It varies by state, and there's no single national threshold. Some states set it at three to five unexcused absences in a school year, others count three consecutive unexcused days, and some measure chronic absenteeism as missing 10 percent or more of the school year for any reason. Confirm your specific state's definition rather than assuming a standard number applies everywhere.
Consequences can reach both the student and the parents. Parents can face fines that commonly start at $25 to $100 for a first offense but can reach $500 to $1,000 or more in some states, plus court fees and, in severe cases, jail time. Students can face consequences including delayed or suspended driving privileges, community service, or court-ordered monitoring. Courts generally treat parents who engage cooperatively far more leniently than those who ignore notices.
Capture enough to make daily records accurate and defensible later:
- Student name and identifying information
- Date and attendance status (present, absent, tardy)
- Whether an absence is excused or unexcused, and the reason
- Any supporting documentation attached
- Staff member recording the entry
Yes. Typeform's signature question lets a parent or staff member sign by typing, drawing, or uploading their signature, time-stamps it the moment they submit, and locks the response so it can't be changed afterward, generating a PDF you can keep on file. It's available on Typeform's Growth Flow, Growth Custom, and Enterprise plans.

College Scholarship Application Form
An application submitted as a PDF from one student and a Google Doc link from another isn't a fair comparison, it's whichever format the committee finds easiest to skim that week.
