Works great on every device.

A lot can happen in 8 hours. This form gives outgoing shift leaders a structured way to document what happened, what's pending, and what the incoming team needs to know — so nothing falls through the cracks at handover.

The template covers the shift date and hours, the reporting supervisor's name, a summary of key activities, any incidents or issues, equipment status, staffing notes, tasks carried over to the next shift, and priority items for immediate attention. Attachments can be added for incident reports or supplementary logs.

When handover information is consistent and documented, the incoming team starts the shift confident — not scrambling to piece together what happened. It also creates an operational log that's useful for reviewing patterns over time.

The shift supervisor or senior team member responsible for the shift. For teams without a formal supervisor, designate a rotating responsibility so everyone knows who completes it each shift.

5 to 10 minutes at most. Keep it structured and focused on what the next shift actually needs to know. More isn't always better — clarity matters more than length.

Yes. The form works for any shift type. Add a shift type field to differentiate between day, evening, and overnight reports in your records.

Retention requirements vary by industry. For healthcare and manufacturing, specific regulatory requirements may apply. In general, keeping 90 days of shift reports provides enough history to investigate incidents or spot operational trends.
