Collect the context that makes a reference useful:

  • Position, organization, and industry they're applying to
  • Brief description of the role and what it involves

  • Format required (written letter, online form, phone call)

The form itself doesn't solve this problem, but having a clear request on record makes it easier to have an honest conversation. If a request comes in and you don't feel confident giving a strong endorsement, it's better to respond promptly and explain that than to submit a lukewarm reference that doesn't serve the person.
