Informed consent in counseling is both an ethical obligation and the foundation of a trustworthy therapeutic relationship. Clients need to understand what counseling involves, what to expect from you as a practitioner, the limits of confidentiality, how records are managed, what happens in a crisis, and their right to end treatment — all before the first session begins.

This template covers the counselor's qualifications and theoretical approach, the nature of the counseling process and its limitations, session structure and fees, cancellation and no-show policy, confidentiality and its legal limits (mandated reporting, imminent danger, court orders), electronic communication policy, what happens if the client is in crisis between sessions, and a signature confirming the client has read, understood, and agreed to the terms.

Licensed counselors, therapists, psychologists, and social workers in private practice and agency settings use informed consent forms before the first session with every new client. Send the link as part of new client onboarding. Store signed consents in the client file. Review relevant sections whenever policies change and obtain updated consent as required by your licensing board.
