A review collected at the right moment, with the right questions, is worth far more than a star rating submitted on a platform you don't control. Your own product review form gives you structured feedback that's tied directly to purchase data, can be displayed on your site exactly as you want, and captures the details that help future buyers make a decision.

This template collects the reviewer's name and email, an overall rating, ratings on specific dimensions (quality, value, packaging, delivery speed, and so on depending on your product), a headline, the review body, and a photo upload for visual products. Optional fields for how the reviewer found you and what they would improve round out the qualitative picture.

Trigger it automatically via email a set number of days after delivery so the request is timely. Responses can populate your site's review section directly or sync to a review management platform. Either way, you own the data and aren't dependent on third-party review policies.

After the customer has had enough time to actually use the product — typically 7–21 days after delivery for most physical goods. For subscription products or services, a review request after the first full billing cycle captures more considered feedback. Asking too soon gets ratings on packaging and delivery, not the product itself.

Yes. Reviews submitted through your own form are yours to use — you're not subject to third-party platform terms. Make sure your form terms or the confirmation screen note how reviews may be displayed, so customers know their submission may be published.

Don't delete or ignore them. A mix of positive and critical reviews actually improves trust — perfect reviews look manufactured. For negative reviews, consider reaching out to the reviewer to resolve the issue, and where appropriate, note your response publicly on the review display. Recovery from a bad experience is a powerful trust signal.

This varies by platform and jurisdiction. For reviews displayed on your own site, a small incentive (discount, loyalty points) is generally fine as long as you're transparent about it and don't make positive reviews a condition of receiving it. On third-party review platforms like Google or Amazon, incentivized reviews violate terms of service.

Connect the review request trigger to your order management system so requests are only sent to confirmed buyers. In the form, include the order number as a pre-filled field that links the review to a specific transaction. This prevents incentivized fake reviews and gives reviews a 'verified purchase' credibility signal.
