The template covers the parent's name and child's name, the area or program they're commenting on, a rating of their overall experience, open-ended feedback about what's working and what could improve, and an optional question about how they'd like to be contacted for follow-up.
Because the form is structured, you can spot trends across responses rather than managing scattered emails and phone calls. When the same issue shows up in multiple submissions, you'll see it.
After major milestones like the end of a term, parent-teacher conferences, or a special event. Some organizations also run quarterly or annual surveys. The key is consistency — irregular surveys get lower response rates.
Keep it short and mobile-friendly. Parents are busy. A 5-minute form completed on a phone during school pickup is more valuable than a 20-minute survey nobody finishes.
You can offer the option. Fully anonymous feedback tends to be more candid, but identified feedback allows you to follow up individually when needed. Consider making name fields optional.
