HOA meetings rarely achieve full attendance, but decisions need to be made. A proxy form lets homeowners formally authorize another person — or the board itself — to vote on their behalf, ensuring quorum is met and every voice has a path to representation even when schedules don't allow for in-person participation.

This template captures the homeowner's name and property address, the name of the designated proxy, the specific meeting or vote it applies to, and a digital signature confirming the authorization. Optional fields let homeowners specify how they want their proxy to vote on specific agenda items, rather than leaving it entirely open-ended.

The form can be shared via email link ahead of the meeting, completed on any device, and submitted in minutes. Responses are stored digitally and can be exported before the meeting starts so the board knows exactly who is represented by proxy before counting begins.

In most U.S. states, digital proxies are recognized under electronic signature laws, but specific requirements vary by state and may also depend on your HOA's governing documents. Check your CC&Rs and local statutes to confirm what format is required and whether digital submission is explicitly permitted.

That depends on how the proxy is structured. You can design the form to give the proxy broad authority to vote as they see fit, or you can include fields that let the homeowner specify their preferred vote on each agenda item. The latter gives homeowners more control while still allowing representation.

Send it at least 7–10 days before the meeting. Homeowners need time to review the agenda, decide how they want their vote directed, and submit the form before the cutoff. Many HOA bylaws also specify minimum notice periods for proxy submission.

Most HOA bylaws place a limit on how many proxies one person can hold to prevent disproportionate control. Check your governing documents before allowing unlimited proxy accumulation — and consider noting the per-person limit on the form itself.

Retain completed proxy forms with the meeting records. If a vote result is challenged, the proxy forms are the documentation that confirms each vote was properly authorized. Keep them for at least as long as your governing documents require board minutes to be retained.
