Grant Application Form Template

Works great on every device.

Reviewing grant applications is already a demanding process. When applications arrive in inconsistent formats — some as email attachments, others as PDFs with missing sections, evaluators spend more time organizing than evaluating, and applicants waste time guessing what you need.

This grant application form template provides a structured, guided format that walks applicants through each required section: organization background, project description, goals and outcomes, budget, timeline, and supporting documentation. Conditional logic adjusts requirements based on the grant type or funding amount, so small grants don't require the same depth as major awards.

Integrate with your review platform, spreadsheet, or project management tool so applications flow directly into your evaluation workflow. Evaluators get consistently formatted submissions, and applicants get a clear, fair process.

Grant Application Form Template FAQs:

A grant application form is the standardized document organizations use to collect and evaluate funding requests. It typically captures the applicant's background, project proposal, budget, timeline, and expected impact. Having a consistent format ensures every submission gets a fair review against the same criteria.

Paper and email-based applications create inconsistency — different formats, missing sections, illegible handwriting, and attachments that don't open. An online form ensures every applicant provides the same required information in the same structure. It also makes the review process more efficient — evaluators can filter, sort, score, and compare applications without reformatting anything.

  • Organization name, mission statement, and tax-exempt status
  • Project title and detailed description
  • Total budget with line-item breakdown
  • Project timeline and key milestones
  • Expected outcomes and how impact will be measured
  • References or letters of support (file upload)

Assemble a review committee with diverse expertise relevant to your funding areas. Develop a scoring rubric before applications open, with weighted criteria for impact, feasibility, budget reasonableness, and organizational capacity. Have each application reviewed by at least 2-3 evaluators independently, then discuss top-scoring proposals as a group. Document decisions and provide feedback to unsuccessful applicants when possible — it builds goodwill and strengthens future applications.

A letter of inquiry (LOI) is a preliminary screening step — a short summary (1-2 pages) that lets funders assess whether a project aligns with their priorities before requesting a full application. A full application is the detailed proposal with budget, timeline, and supporting documentation. Using an LOI stage saves time for both funders and applicants by filtering out misaligned proposals early. Consider implementing a two-stage process if you receive a high volume of applications.
