Hiring drivers — whether for deliveries, transportation, or fleet operations — requires more documentation than a standard employment application. Driving record, license status, and experience history are just the start. This form gives employers a structured way to gather everything they need before a hire decision is made.

The template covers the applicant's name and contact information, driver's license class and expiration, license number and state, driving history over the past 3 to 5 years, accident and violation record, employment history for driving positions, reference contacts, and consent to a motor vehicle record check.

Motor vehicle record (MVR) check, criminal background check, employment history verification, and for commercial drivers, a pre-employment drug and alcohol test and DOT background check through the FMCSA Clearinghouse.

At minimum 3 years for most positions. For commercial or DOT-regulated roles, FMCSA requires 10 years of employment history and 3 years of MVR data. Adjust based on the risk profile of the role.

This varies by employer policy, insurance requirements, and the nature of the role. Common disqualifiers include DUI convictions, license suspensions, excessive violations, and major at-fault accidents within recent history. Establish clear, written criteria.

Employment law requires individualized assessment, not automatic exclusion. For older violations, consider the nature of the offense, time elapsed, evidence of rehabilitation, and relevance to the specific driving role. Consult your attorney on your specific policy.

An MVR is a report from the state's DMV that shows a driver's license status, endorsements, violations, accidents, and suspensions. Run MVRs before hire and periodically for current drivers — some insurers require annual monitoring.
