An alumni registration form collects updated contact information and engagement preferences from former students, employees, or members of an organization. It's used to build and maintain an accurate alumni database, segment communications, and facilitate network engagement.

Your alumni network is a long-term asset, but only if you can reach people. A registration form gives alumni an easy way to stay connected on their own terms, and gives your organization accurate, current data for outreach, fundraising, events, and mentorship programs.

Cover contact details and engagement preferences:

• Full name (including name at time of graduation/departure)
• Current email and phone number
• Graduation year or membership period
• Current employer and job title
• Location
• Areas of interest (mentoring, events, donations, volunteering)
• Preferred communication frequency

Connect the form to something of value — access to an alumni directory, an event invitation, or a mentorship program. People are more likely to share their information when they understand what they'll get in return. A brief, personal note from a well-known alumni or leader can also increase response rates significantly.

Send a short update form annually and ask alumni to confirm or update their details. This is much easier than running a full re-registration campaign from scratch. You can also include a self-service link in every email communication so alumni can update their preferences whenever something changes.
