Film, video, and media projects that use actors without a signed release face distribution problems, legal disputes, and delays when it's time to sell or screen the work. This form gives productions a clean way to obtain and document the rights they need upfront.
The template covers the actor's name and contact information, the project name and description, the role or performance being released, the rights granted (synchronization, distribution, streaming, derivative works), any restrictions, compensation terms, and a dated signature. No. A parent or legal guardian must sign on behalf of a minor. Include a guardian section and note that releases signed by minors themselves are not enforceable.
You can't use the performance without a release. Casting contracts typically require release signing as a condition of participation. Address this in the booking agreement rather than on the day of the shoot.
