5S Audit Form

A 5S audit form evaluates a workspace against the five lean methodology pillars and produces a score for each one. It evaluates Sort, Set in Order, Shine, Standardize, and Sustain across defined work zones using a consistent scoring scale. Think of it as the record that shows management where the 5S standard holds and where it needs attention.

5S audits only create value when they're conducted consistently and recorded objectively. When audits vary by auditor or aren't recorded consistently, management has no reliable data to act on. A complete 5S audit covers:

• Facility, department, or work zone name
• Audit date and auditor name
• Sort: unnecessary items removed, storage areas clear, red-tag process in place
• Set in Order: tools and materials labeled and returned to designated locations, visual management in place
• Shine: equipment, floors, and surfaces clean, cleaning schedules followed, no leaks or spills unaddressed
• Standardize: standard operating procedures posted and followed, visual standards in place, responsibilities assigned
• Sustain: previous audit actions completed, team engagement visible, continuous improvement activities active
• Overall score per pillar (1–5 scale)
• Any non-conformances identified and corrective action assigned
• Auditor signature and next audit date
